Transition to Growing Kids Concierge

The Concierge practice is a membership-based practice accepting commercial/private insurance for families who are tired of paying high deductibles or facing uncertain healthcare costs.
We are still working through the details for our Medicaid patients but expect the number of patients to decrease given the decreased size of the practice. We will work with all Medicaid patients that do not stay with the practice to transition care to your new provider. Those Medicaid patients that stay with the practice will continue to be covered by their Medicaid plan of choice. Medicaid patients do not pay cash for care.
We will continue to operate as normal through July and August, with all existing appointments being honored and all insurance and Medicaid being accepted. During that time, Growing Kids Pediatrics will be available for new appointments based on availability and emergencies or consultation as usual. I am not interested in a concierge service
If you choose not to join our concierge service, we understand and respect your decision. We will provide recommendations for pediatric practices in the region that accept traditional insurance. Our goal is to make the transition simple and easy for you and your child. We recommend the following practices that we are familiar with:

How do I receive medical records and how long will I be able to access them?
You will need to fill out a records release from GKP (see downloadble form below) or at your new pediatric office. GKP has 30 days from the date we receive your records release to transfer your child’s records.
How can I get more information or ask specific questions?
We will have information sessions at GKP throughout July and August to answer any questions you may have. These dates will be posted and more information will be available on our website: growingkidspediatrics.com.

Why is Growing Kids Pediatrics moving to a hybrid concierge model?
We understand in the current economic climate that many families are not able to move to a cash-based only model for their healthcare, which many other practices in the region have already moved to. Dr. Lane believes that our community deserves the benefits of personalized, comprehensive care while still accepting insurance. The hybrid model that we are adopting allows for this.
Growing Kids Concierge is a new approach to pediatric care designed to provide the highest quality service and convenience for your family. With the concierge practice you will have 24-7 access to me – Dr. Christy Lane – and my team, ensuring that your child’s health is always our top priority.

What are the membership fees?
Membership fees provide increased access to Dr. Lane and more personalized, comprehensive care, while insurance covers visits and treatments.

Membership fees can be paid monthly, quarterly, or annually depending on what works best for your family. Your membership fees may even qualify as eligible expenses under your Flexible Spending Account (FSA) and/or Health Savings Account (HSA) plans. Be sure to review your individual plan to determine what costs may apply.
Will membership fees differ based on the age of the child?
How many patients will you be seeing as part of Growing Kids Concierge?
Growing Kids Concierge will be significantly smaller than the existing practice. We will determine the number of patients that we can accommodate to maintain the highest quality of service.
I have an appointment scheduled in July or August or need to schedule an appointment in July or August. Are you still seeing patients? Are you still taking insurance?
Yes. Existing appointments will be honored, and existing patients can continue to schedule appointments based on availability. Growing Kids Pediatrics’ office remains open regular hours through the end of August:
Monday-Thursday: 9 am – 5 pm
(closed 12-1 pm for lunch)
Friday: Office is open for calls
We will continue to accept all private/commercial insurance and Medicaid during this time.
Will you still bill my insurance directly? What about the cost of labs, x-rays, or other services provided by third parties?
The membership fees do not replace your insurance. Dr. Lane will continue to bill your insurance for all insured health care services provided by Growing Kids Concierge and Dr. Lane. Your membership fee will offset co-pays that you normally would pay at the time of service.
The cost of services provided by a third party (an entity other than Growing Kids, such as LabCorp or other specialists), including hospitalizations, emergency room visits, prescription medications, outside lab work, x-rays, and evaluations by other physicians or specialists will continue to be handled in the same manner as before – with Dr. Lane coordinating care, prescribing medicine, ordering tests, and facilitating care by other physicians or specialists where needed while the third party is responsible for billing your insurance.
Is the membership fee a medical expense that … (a) is eligible for reimbursement as a medical expense from my FSA or HSA, or (b) can be used in calculating itemized deductions?
(a) Whether the membership fee is fully reimbursable as a medical expense is dependent upon your particular plan. If not fully reimbursable, in most cases it will qualify as a medical expense for the amount of any out-of-pocket costs to GKC that the membership fee is used to offset.
(b) IRS Publication 502 explains the itemized deduction for medical and dental expenses. We recommend speaking with a qualified tax professional.
Does the membership fee count towards my deductible?
For most insurance plans, the membership fee does not count towards the deductible except for that portion which is used to offset your out-of-pocket costs to GKC or “co-pay.” All such out-of-pocket costs will be reported as paid as of the date the service is provided.
Can you provide me with examples of how the use of the membership fee and insurance looks in practice?
- As a founding patient of GKC, you pay a monthly membership fee of $150 for your one child. Your health insurance plan does not require a co-pay for this visit, but the provider identifies an issue or you raise an issue that is not part of a regular wellness check-up that needs to be addressed during the visit requiring ordering of additional tests and referring your child to a specialist.
- The co-pay of these additional services conducted in-house at GKC would normally be $100, which is offset in its entirety by your membership fee.
- Your insurance is billed as normal and notified that you have made your co-pay. This $100 would then count towards your deductible and, in most cases, be reimbursable under your FSA or HSA as of the date of the service.
- GKC assists with identifying and getting you an appointment with a specialist. Although you will be responsible for any co-pays associated with the specialist (as that is not a service provided by GKC), GKC is able to stay actively engaged with the consult and able to see you almost immediately after discussing the case with the specialist to discuss the results and any treatment plans or recommendations. Any co-pay payable to GKC for this follow-up visit would again be off-set by your membership fee. If regular follow-up visits are required with GKC to manage medication or any treatments, co-pays for those follow-up visits would also be off-set by your membership fee (and once again, counts towards your deductible and be reimbursable as of the date of the service).
- Your child spikes a high fever at 7 pm on Friday night and you are certain it is strep. You text your provider at GKC and ask what you should do. Rather than requiring a visit to urgent care on Friday night or Saturday morning, where your co-pay and a strep test could be more than $250, GKC meets you at the office or stops by your house to check on your child and run the strep test. Any co-pay for the sick visit and strep test would be off-set by your membership fee (and once again, counts towards your deductible and be reimbursable as of the date of the service). You also saved yourself additional out-of-pocket costs and hours of waiting in a waiting room at an urgent care facility to see an unfamiliar provider.
- You are concerned that your child seems off, but aren’t quite sure what is wrong. You normally wouldn’t take your child to the doctor’s office because it’s a hassle to get an appointment and you don’t want to pay yet another co-pay if nothing turns out to be wrong. Because you are a patient of GKC, you schedule an appointment just in case. During the visit, GKC identifies the early stages of an ear infection and is able to begin treatment before the infection becomes more severe. Your insurance is billed and your co-pay is once again offset by your membership fee (and once again, counts towards your deductible and be reimbursable as of the date of the service).
What if my child is healthy this year and I only need my well-child visit? Would I be able to “roll-over” the membership fee that was not used to offset my out-of-pocket costs?
No. The membership fee is only applicable for the then-current 12-month membership period and will not carry over. Although you may not need to see GKC for sick visits, you are still able to enjoy the additional benefits offered by GKC, such as quarterly parenting classes, unlimited camp or school medical forms, and teen classes on topics like skincare. Efforts will be made to provide itemized invoices for the value of any utilized services that generally would be considered entirely out-of-pocket by your insurer, but that are offset by your membership fee, allowing you to pursue reimbursement for all that are eligible under your HSA or FSA.
What if my child is really sick this year and the total cost of our co-pays to GKC would be more than the membership fee? Would we then owe GKC more?
No. The membership fee will offset all co-pays owed to GKC.
Note, however, you will need to consult with your insurer to determine whether you can claim the offset co-pays that exceed the annual membership fee towards your annual deductible.
